The TFSA started in 2009. You accumulate room for each year in which you are 18 or older and a Canadian resident. If you turned 18 in 2010, your room starts from 2010; earlier years do not count for you.
Yes. Withdrawals from your TFSA are added back to your contribution room in the following calendar year. The calculator accounts for withdrawals when you enter them to show your room as of the end of your chosen year.
